Jersey County Emergency Management Agency encourages residents to use File of Life
Emergency responders want to help you as quickly and safely as possible. One of the fastest ways to do that is by having critical medical and emergency information readily available.
The File of Life program allows residents to keep important information in a clearly marked location—typically on the refrigerator—where first responders are trained to look during an emergency.
What Information Should Be Included?
■ Residents are encouraged to complete the File of Life form with:
■ Medical conditions
■ Medications & dosages
■ Allergies
■ Emergency contacts
■ Physician information
■ Medical instructions
Why it matters
In an emergency, you may not be able to speak for yourself. Phones may be locked,
uncharged, or unavailable. A completed File of Life allows first responders to:
